What bugs/issues did you encounter:

 I have swapped my director of football. When i look at the people they are assigned to the right clubs, however when i look at the clubs and when receiving feedback/assigning tasks the old staff is mentioned.

To be honest, the Swap Staff feature should be used with extreme caution and as a last resort.  It is best that you revert back to a previous save before you did the swap.
